Change Agent/Community Health Worker Job Description
FLSA Status: Non-Exempt
Reports to: Director of Reentry Services
Job Summary:
The Change Agent/Community Health Worker helps individuals and communities adopt healthy behaviors and conducts outreach to implement programs in the community that promote, maintain, and improve health outcomes.
Supervisory Responsibilities:
None.
Duties/Responsibilities:
Work closely with the Department of Adult Corrections to collaborate on re-entry planning.
Conduct Individualized Re-entry Plan (IRP) pre and/or post release.
Facilitate resource navigation.
Provide culturally aligned and responsive recovery coaching services.
Identifies or contacts members of high-risk or otherwise targeted groups such as members of minority populations or low-income populations.
Assist members in obtaining vital records and driver’s license restoration.
Advises clients or community groups on issues related to improving general health, including physical health, mental health, public health, and risk prevention.
Attend community meetings or health fairs to understand community issues or to build relationships with community members. Distributes flyers, brochures, or other informational or educational documents to inform members of a targeted community about available resources.
Collect information from individuals to compile vital statistics about general health or community members.
Advise members and community groups on issues pertaining to returning citizens and impacted families.
Conducts home visits for high-risk individuals to monitor their progress or to assess their needs.
Transports or accompanies clients to scheduled health appointments or referral sites such as recovery meetings and support groups.
Advocates for individual or community health needs with government agencies or health service providers.
Teaches appropriate parenting behaviors to individuals or families.
Assists families to apply for social services including Medicaid or Women, Infants, and Children (WIC).
Interprets, translates, or provides cultural mediation related to health services or information for community members.
Record member encounters and appropriate information in Apricot system in a timely manner.
Retains and submits receipts for all spending on behalf of client or Operation Gateway promptly.
Performs other duties as assigned.
Required Skills/Abilities:
Knowledge of community health resources.
Understand reentry processes and demonstrate cultural sensitivity.
Ability to effectively communicate with a diverse population and establish trust.
Effective time management skills and ability to prioritize tasks.
Familiarity with electronic devices, MS Office suite, and ability to learn Apricot Database System.
Typing skills
Effective active listening skills and social awareness.
Education and Experience:
Community Health certification required.
Peer Support Specialist Certification.
Three to five years of related or lived experience required.
Physical Requirements:
Prolonged periods of sitting at a desk and working on a computer. May be required to stand for prolonged periods of time, climb stairs, and perform strenuous activity.
Must be able to lift 50 pounds at times.
NC Driver's License, Automobile Insurance and reliable transportation required.
